A key part of the NHS Long Term Plan is the development of Primary Care Networks (PCNs), which seeks to bring practices together to work at scale.
They are expected to be the footprint around which integrated community-based teams will develop; changing the shape of primary care to focus their role on supporting and building healthy communities. Therefore the role of the PCNs and that of the delivery of integrated care locally will have clear links to be further considered.
